Two major therapy changes by Dr. Obagi:

1) Dr. Obagi now advocates a "New Pulse Therapy of Hydroquinone (HQ)" so that the patient goes off completely after 3 months or so. Long term use of HQ makes skin photosensitive, causing inflammation which results in what he terms "weird pigmentation".

My question is how to stabilize the melanocytes so that you don't need HQ yet the melasma doesn't return?

AHAs and lactic acids and lighteners Kojic acid, azaleic acid, arbutin have not stopped the melasma from returning.

2) He also says continuous use of tretinoin causes irritation and damage.

What do you use in place of tretinoin? And how long are you supposed to take breaks from tretinoin? I thought you have to use tretinoin consistently to get anti-aging effects.

I am on Obagi NuDerm maintenance and having rebound pigmentation. If I increase HQ application to 4 x a week, it stops. Anything less, the darker areas reappear.

Are there any patients of Dr. Obagi being treated for melasma who can comment on his new regime? Would greatly appreciate your comments on what you are doing now on maintenance.

Dr. Obagi replaces HQ with Brightenex (retinol with ascorbic acid, antioxidants and "Multistage glutathione, soy isoflavones, and N-acetyl glucosamine melanin inhibitor system") and tretinoin with Retamax (mainly retinol).

Has anyone tried these products or can recommend substitutes (I can't get them where I live).

Why not just use a low strength tretinoin instead of Retamax?

Dr. Obagi only uses tretinoin and HQ for 5 months then stop for 2 or 3 months or until needed. I'll switch to Skinceuticals 1.0 Retinol and see what happens.

Wow you have researched a lot. Good information.
But what should we have to use in place of obagi?
Still question.. I never used Zo products. I don't know what to do..
